Pennsylvania Drivers License History Request Check List

Please fill out Pennsylvania Drivers License History Form DL-503 with the following:

• Check 10 YEAR DRIVER RECORD.
• Leave Section A: REQUESTER INFORMATION blank; this section will be completed by County House Research, Inc.
• Complete Section B: END USER INFORMATION BEING REQUESTED; provide CRA/Investigator information.
• Complete Section C: DRIVER INFORMATION; print clearly and boldly.
• Check E = EMPLOYMENT in Section D: AFFIDAVIT OF INTENDED USE;
  The Request will be printed and signed by our office.
  Notarization will be completed by County House Research, Inc.
• Complete Section E: DRIVER RELEASE; have the applicant complete this section and sign clearly.
  Forms lacking a clear signature will not be processed.
• Leave Section F: MICROFILM blank.

Please note: Incomplete forms and forms missing a signed release will not be processed.
